A $70.00 dispatch fee will be charged for a la carte orders provided at customer locations for all requests with less than five employees. A $70 cancellation fee will be charged for any scheduled service that is canceled with less than 24 hours notice. On Site Medical requires request for scheduled service to be scheduled 10 days prior to date of services to guarantee availability. On Site Medical will require access to a bathroom to administer drug tests. On Site Medical will require access to a private, quiet space for conducting all screening services.
For CPR training, mannequins provided will be one for every four attendees. CPR classes are pre-paid at the time of scheduling. Payment for additional attendees not originally scheduled will be due within 72 hours of completion of training. Refunds will not be provided for no-show attendees. Pre-paid classes that are canceled at least 5 business days prior to scheduled training will be refunded. Any classes canceled within 5 business days of scheduled training will result in a credit for future training. Classroom environment must be provided at your location (e.g. training center, conference room, large office with ample seating, etc.) Training area must be indoors and have power outlets available.
